Properties:
Torasemide is a “loop” diuretic. The main mechanism of action of the drug is due to the reversible binding of torasemide to the sodium/chlorine/potassium ion cotransporter located in the apical membrane of the thick segment of the ascending loop of Henle. As a result, the reabsorption of sodium ions is reduced or completely inhibited and the osmotic pressure of the intracellular fluid and the reabsorption of water are reduced. Blocks aldosterone receptors in the myocardium, reduces fibrosis and improves diastolic myocardial function.
Torasemide, to a lesser extent than furosemide, causes hypokalemia, while it exhibits greater activity and its effect is longer.
The maximum diuretic effect develops 2-3 hours after taking the drug inside.
Torasemide can be used for a long time.
Indications:
– Edema syndrome of various origins, incl. With chronic heart failure, diseases of the liver, kidneys and lungs;
– Arterial hypertension.
Dosage and administration:
The drug is taken orally 1 time per day before or after meals with a small amount of water.

The therapeutic dose is 5 mg 1 time per day. If necessary, the dose should be gradually increased to 20-40 mg 1 time per day. The maximum single dose is 40 mg, it is not recommended to exceed it (there is no experience of use). The drug is prescribed for a long period or until the disappearance of edema.
Contraindications:
Anuria; hepatic coma and precoma; refractory hypokalemia; refractory hyponatremia; dehydration; pronounced violations of the outflow of urine of any etiology (including unilateral lesions of the urinary tract); glycoside intoxication; acute glomerulonephritis; sinoatrial and AV block II and III degree; children and adolescents up to 18 years of age; pregnancy; hypersensitivity to torasemide; allergy to sulfonamides (sulfonamide antimicrobials or sulfonylureas).
Precautionary measures:
In patients, especially at the beginning of treatment with torasemide and in the elderly, it is recommended to monitor the electrolyte balance, volume and concentration of circulating blood.
With long-term treatment with torasemide, regular monitoring of electrolyte balance (especially potassium levels), glucose, uric acid, creatinine, lipids and cellular blood components is recommended.
Side effects:
From the nervous system: often – dizziness, headache, drowsiness;
From the digestive system: often – diarrhea;
From the side of the kidneys and urinary tract: often – an increase in the frequency of urination, polyuria, nocturia.

Store at a temperature not exceeding 30 degrees. In a dry place.
