Losartan potassium — 50 mg per tablet.
Description
LOSAR is an antihypertensive medication belonging to the group of angiotensin II receptor antagonists (type AT1). It effectively lowers blood pressure, improves kidney function in diabetic patients with proteinuria, reduces the risk of stroke in patients with left ventricular hypertrophy, and is also used in chronic heart failure when ACE inhibitors are not tolerated.
Indications for Use
-Treatment of essential hypertension in adults and children aged 6–18 years.
-Renal protection in hypertensive adult patients with type 2 diabetes mellitus and proteinuria ≥ 0.5 g/day.
-Treatment of chronic heart failure in adults when ACE inhibitors are not tolerated.
-Reduction of stroke risk in hypertensive adults with left ventricular hypertrophy.
Dosage and Administration
Adults:
Usual starting dose: 50 mg once daily.
May be increased to 100 mg once daily if needed.
For heart failure: start with 12.5 mg and titrate up to 150 mg daily.
In diabetic nephropathy:
Initial dose: 50 mg once daily, may be increased to 100 mg if necessary.
Special populations:
Elderly: start with 25 mg if over 75 years of age.
Hepatic impairment: use with caution; contraindicated in severe cases.
Renal impairment: no dose adjustment required.
Children aged 6–18: 25–50 mg depending on body weight.
Not recommended in children under 6 years or with GFR <30 ml/min/1.73 m².
Method of administration:
Taken orally with water, with or without food.
Contraindications
-Hypersensitivity to losartan or any component of the formulation.
-Severe hepatic impairment.
-Pregnancy (2nd and 3rd trimesters).
-Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 ml/min/1.73 m²).
Precautions
-Dual blockade of the renin-angiotensin system (with ACE inhibitors or aliskiren) is not recommended.
-Risk of hyperkalemia, particularly in renal impairment.
-Monitor potassium and creatinine levels in patients with CHF and GFR 30–50 ml/min.
-Use caution in patients with hypovolemia, diarrhea, vomiting, or on diuretics.
-Reduced antihypertensive effect may occur in Black patients.
Side Effects
Dizziness, hypotension.
Hyperkalemia, hypoglycemia.
Impaired renal function.
Cough, angioedema.
Fatigue, headache.
Rare: rash, pruritus, abdominal pain.
Pregnancy and Lactation
Contraindicated in the 2nd and 3rd trimesters due to risk of fetal injury or death. Alternative medications should be used when planning pregnancy.
Use during lactation is not recommended due to lack of safety data.
