Cymbatex 30 mg 30 hard gelatin capsules Composition: Each hard gelatin capsule contains: Duloxetine hydrochloride 33.65 mg (equivalent to 30 mg duloxetine) Description: Cymbatex is a delayed-release capsule containing duloxetine, a serotonin and noradrenaline reuptake inhibitor (SNRI). It exhibits weak inhibition of dopamine reuptake and has negligible affinity for histaminergic, muscarinic, or adrenergic receptors. Duloxetine increases extracellular levels of serotonin and noradrenaline, helping to modulate the pain pathways in the central nervous system and improve mood in depressive and anxiety disorders. Indications for Use: Major depressive disorder (MDD) Diabetic peripheral neuropathic pain Generalized anxiety disorder (GAD) in adults Dosage and Method of Administration: MDD: Recommended dose is 60 mg once daily. Maximum: 120 mg/day. GAD: Start at 30 mg once daily, increase to 60 mg if needed. Maximum: 120 mg/day. Diabetic Neuropathy: Start and maintain at 60 mg/day. May increase to 120 mg/day in divided doses. Elderly patients: No dosage adjustment required, but use with caution at higher doses. Children: Not recommended. Hepatic impairment: Contraindicated. Severe renal impairment (GFR < 30 ml/min): Avoid use. Discontinuation: Gradual dose reduction over 1–2 weeks is advised. Route of administration: Oral use. Contraindications: -Hypersensitivity to duloxetine or any excipient -Use with non-selective, irreversible monoamine oxidase inhibitors (MAOIs) -Liver disease with hepatic impairment -Concomitant use with potent CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin) -Severe renal impairment (GFR < 30 ml/min) -Uncontrolled hypertension Precautions: -Use with caution in patients with a history of mania or bipolar disorder, seizures, increased intraocular pressure, or cardiovascular disease -Monitor blood pressure and heart rate regularly -Avoid abrupt discontinuation -Risk of suicidal thoughts and behavior, particularly at treatment initiation -Risk of hyponatremia, especially in elderly or patients taking diuretics -Risk of serotonin syndrome, akathisia, and SIADH -Not recommended for use in children and adolescents Side Effects: Very common: Nausea, dry mouth, headache, somnolence Common: Insomnia, dizziness, decreased appetite, constipation, fatigue, blurred vision, increased sweating Uncommon/Rare: Suicidal thoughts or behavior, mania, mydriasis Hepatotoxicity, hyponatremia, Stevens-Johnson syndrome Aggression, hallucinations, sexual dysfunction Orthostatic hypotension, urinary retention, weight changes Withdrawal symptoms: agitation, dizziness, insomnia Pregnancy and Breastfeeding: Not recommended during breastfeeding. Duloxetine is excreted into breast milk in small amounts. Limited data are available on safety in infants. Use during pregnancy only if the potential benefit justifies the risk. Storage Conditions: Store in a dry place at a temperature not exceeding 30°C. Keep out of reach of children. Packaging: Carton box containing 3 blisters of 10 hard gelatin capsules each.
