Composition: Each tablet contains: Active ingredient: Metformin hydrochloride 500 mg. Inactive ingredients: Polyvinylpyrrolidone K90, Talc, Ethanol 96%. Description: Cidophage is an oral hypoglycemic agent of the biguanide class used to treat type 2 diabetes mellitus, particularly in overweight patients when diet and exercise alone fail to achieve adequate glycemic control. Metformin reduces plasma glucose levels by improving insulin sensitivity and decreasing hepatic glucose production without causing hypoglycemia. Indications for Use: Treatment of type 2 diabetes mellitus, especially in overweight patients, when dietary management and exercise are insufficient. May be used as monotherapy or in combination with other oral antidiabetic agents or with insulin in adults, and in children aged 10 years and older. Dosage and Method of Administration: - Take orally during or after meals. - The usual starting dose is 500 mg two to three times daily.After 10-15 days, the dose should be adjusted based on blood glucose levels. - Maximum daily dose in adults - 3000 mg (in 2-3 divided doses). - For children from 10 years of age - maximum daily dose 2000 mg. - When switching from another oral antidiabetic drug, discontinue the previous medication and start metformin at the standard initial dose. Contraindications: - Hypersensitivity to metformin or any excipients. - Severe renal failure (GFR < 30 ml/min). - Acute metabolic acidosis (including lactic acidosis and diabetic ketoacidosis). - Acute conditions affecting renal function (dehydration, shock, severe infection). - Conditions causing tissue hypoxia (cardiac or respiratory failure, recent myocardial infarction). - Hepatic insufficiency, alcoholism, or acute alcohol intoxication. Precautions: - Lactic acidosis: a rare but serious complication, usually due to acute renal impairment, hypoxia, or sepsis. If symptoms such as acidotic dyspnea, abdominal pain, cramps, weakness, or hypothermia occur, stop treatment and seek immediate medical attention. - Temporarily discontinue metformin in case of dehydration, severe infection, or before contrast imaging or surgery. Restart at least 48 hours later after renal function reassessment. - Caution in elderly patients and those with impaired renal or cardiac function. - Avoid alcohol intake due to increased risk of lactic acidosis. - Maintain a balanced diet with consistent carbohydrate intake. Side Effects: - Very common - gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal pain, loss of appetite), usually transient. - Common - taste disturbance. - Very rare - decreased vitamin B12 absorption, megaloblastic anemia, abnormal liver function tests or hepatitis, skin reactions (erythema, pruritus, urticaria), lactic acidosis. Pregnancy and Lactation: Insulin is preferred during pregnancy to maintain optimal glucose control. Metformin should be used only if the potential benefit outweighs the risk. Metformin is excreted in breast milk; breastfeeding is not recommended during treatment. Storage Conditions: Store below 30°C in a dry place. Keep out of reach of children.
