Trade Name: Polyfresh extra 30 Single dose units × 4 ml Eye lubricant Sterile ophthalmic solution Preservative free Composition: Sodium hyaluronate (0.1 %), Carboxymethyl cellulose (0.5%), Glycerin, Boric acid, Potassium chloride, Calcium chloride, Magnesium chloride, Sodium citrate, Sodium hydroxide, Hydrochloric acid (for pH adjustment) and Water for injection. Properties : Polyfresh extra Single Dose Units is a sterile, preservative-free, clasto-viscous, clear corneal shield. Suitable for contact lens users cases contact lens insertion. Polyfresh extra SDU protects, moistens and lubricates the surface of the eye. It is suitable for disruptions of the delicate balance of the eye’s tear film. Indications: It is suitable for the treatment of the symptoms of dry eye (such as soreness, burning, irritation or dryness) caused by not producing enough tears to keep the eye wet. This includes patients who have Chronic Dry Eye Syndrome and Sjogren’s Syndrome. Polyfresh extra SDU is also suitable for use post LASIK surgery and also for prolonged relief of discomfort associated with mechanical stress, e.g.: Wearing of hard or soft contact lenses. Environmental stress induced by air conditioning, wind, coldness, sun, dryness or smoke. Visual stress from intense viewing, e.g. at computer terminals, microscopes. Advanced age. Directions for use: – Clean your hands and detach a single dose unit container. -Twist off the flat top. Apply 1-2 drops to each eye while pressing lightly. -Apply 1-2 drops into each contact lens to ease insertion and keep the eye moist and lubricated. – Discard the remaining amount after use. – Repeat application 2-4 times daily, as required. Warnings and precautions: – For external ophthalmic use only. – Do not use damaged single dose units. -To prevent bacterial contamination of the vial contents, it is important to avoid touching the tip of the vial to any surface including the eye. If the tip does touch the eye or any other surface then the vial should be discarded. -Do not use it if you are allergic to any of the ingredients. -Do not install Polyfresh extra SDU eye drops at the same time as other ophthalmic drugs. Wait at least 15 minutesbefore the next installment. -Temporary blurring directly after application is possible. Normal vision is generally restored after a short time. -Keep out of the reach of children. Storage: Store at a temperature not exceeding 30°C. Do not freeze. Package: 30 single dose units of 0.4 ml
